THE IMPACT OF PSORIATIC ARTHRITIS ON LONG-TERM WORK ABILITY AND ITS PREDICTORS: A SYSTEMATIC REVIEW

Background

Occupational health physicians assess the potential of an individual’s long-term work disability in the context of an ill-health retirement application. Psoriatic arthritis is a common and debilitating condition; but recent advancements in treatments have implications for long-term work ability.

Aims

To perform a systematic review to explore the impact of psoriatic arthritis on work outcomes and its predictors.

Methods

MEDLINE, Embase, and The Cochrane Library databases were systematically searched for all English language studies from January 2010 to July 2023. Randomised controlled trials (RCTs), cohort studies and case-control studies were included. The quality of included studies was assessed using the Critical Skills Appraisal Programme (CASP) and the Quality in Prognosis Studies (QUIPS) tools. Narrative summaries were used to present the results. PRISMA guidelines were followed.

Results

Seventeen studies were identified (n=15,244). Anti-tumour necrosis factor (anti-TNF) therapies and ustekinumab (UST) significantly improved presenteeism and work productivity, but improvement in employment rates and absenteeism was inconclusive. There was an association between work disability and female gender, older age, longer disease duration, poor physical function, high joint count, work disability prior to treatment initiation, and anti-TNF treatment failure. Low quality evidence suggested work disability may be associated with erosive joint disease, manual work, and poor mental health. Limitations of the studies included heterogenous methods of measuring work outcomes.

Conclusions

Anti-TNF and UST therapies improved measures of work ability (presenteeism and productivity), but improvements in employment rates were minimal. Future research should harmonise work disability measures to facilitate direct comparisons between studies.
